It is optional, when printing prescription discount cards, to provide your name and email address. These fields are provided, as a convenience to you, so that when you print your card it will have your name already populated on the card. Your email address will only be used to send you information and a link on how to reprint your prescription discount in the case it's lost or damaged. This information is never sold or shared with any other party.

There may be times when you "opt-in" to receive additional correspondence from us. In that case, we will not use your email for any other purpose except that which you agree to.

Information about your computer hardware and software collected by DHPOA may include: your IP address, browser type, domain names, access times and referring web site addresses. This information is used by DHPOA to help maintain the quality of the service, and to provide general statistics regarding use of this website. DHPOA keeps track of the internal web sites and pages our customers visit to help determine what plans and services are most popular. We do not share that information with anyone, nor is there any personally identifiable information gleaned from your visits.
